Villa International is a shared residential community. This form is intended to document incidents or ongoing concerns between guests or Villa staff so management can respond appropriately and fairly. Submitting this form initiates a formal review process which will involve multiple staff members and considerable staff time. Please note this information will likely be shared with the other guest to ascertain their side of the story.

If you are in immediate danger, please call 911 or the staff member on duty.

This form is not to be used for small concerns and no action will be taken unless you have tried to solve the problem yourself and talked to the person.

Important Acknowledgements

Villa uses progressive accountability up to and including removal from Villa.
• This report will be filed in the records of all parties involved, including the reporting guest.
• Room changes are limited to a maximum of two per stay. After two room changes, guests may be required to leave Villa.
• Reports must be submitted in good faith. False or retaliatory reports may result in consequences.
• Petty issues are to be handled between guests.
