This form is intended to document incidents or ongoing concerns between guests or Villa staff so management can respond appropriately and fairly. Submitting this form initiates a formal review process which will involve multiple staff members and considerable staff time. False or retaliatory reports may result in consequences. • Petty issues are to be handled between guests.
